• If I complete the form, what am I committing myself to?
  You are agreeing to pay $50.00 deposit which is refundable on installation of your equipment.
  If you elect not to proceed with installation, your $50 is forfeited, otherwise it is credited to your monthly payments, which do not commence until your equipment is installed and working.
  The $50.00 deposit is debited only when the 40 required submissions have been received.
• Section 1 - Select Your Plan and Contract Term
  For most people, the “Budget 500” plan should be adequate.
  You can always upgrade if you find you are exceeding the 500MB quota.
  If you sign up for 36 months, installation is FREE.
  Most people will not require a static IP address - if in doubt, leave the box unchecked.
• Section 2 - Preselect Long Distance Phone
  You must answer Yes or No to this question. If you select No, you don’t need to complete the rest of this section.
• Section 3 - ClearMail account
  Choose a user name and password. You could probably use the ones you have with your current ISP.
• Section 4 - Referral
  Select “Mailbox Delivery” from the drop-down list
  Please put Simon Lister - Koala Communications as the person/company referring you.
  If your current ISP is not listed, select “Other”.
• Section 5 - Existing Customer
  This is self-explanatory.
• Section 6 - Access Details
  Typically, this will be your home address, phone and your name.
• Section 7 - HiBIS Details
  Most people will answer No to both questions.
• Section 8 - Application Details
  You need only complete those fields marked with a red asterisk (*)
• Section 9 - Payment Details
  Depending on which payment method you choose, more fields will appear which you need to complete.
• Sections 10, 11 and 12 - Terms & Conditions
  You must check the box at the bottom of each section.
